Media Services Order Forms
Requests that require the same equipment for the entire semester. If you would like to request equipment for only a few dates throughout the semester, please fill out the Individual Service Order below.

The change order form is used to change equipment, location, time, etc. of previously requested services.
Please note that media services does not process any verbal changes.
This option is for requesting your lecture to be recorded and placed online as a streaming audio or podcast for your students. Only available in lecture halls.
Requests for equipment at the Oakland Center. Must be submitted via fax number on the form or personal drop off at the main administrative office. Any questions may be directed to the phone number on the form.
